Q: Should a cover letter be submitted as a document with a manuscript or as an email?

Answer:
If you are submitting through an online submission system, there will be a separate option for cover letter. The system will ask for different documents to be submitted separately. If, however, you are submitting your manuscript via email, the cover letter should be the body of the email and the manuscript should be added as an attachment.

Either is fine, and you could do both. If submitted with the manuscript, see that the cover letter is placed uppermost. If sent as an email, you could place the text of the cover in the body of the email message and include the manuscript as an attachment to that message.
